Shurmatz Counseling provides different psychotherapy services, here are some of the quick links you can use. If you want to schedule an appointment, simply select on New or Returning client. If you already have set up your appointment and you need to fill out the Patient Intake Form before your schedule, select the Patient Information option below and Patient Information Update if you are to update your existing patient file with us. And, if you looking to pay your amount owed you select the Patient Payment Form.
